2009-10 Youth Ministry Health Form
This is an important form that we use whenever we travel over 2 hrs from home or for any overnight trip. The form must be notarized and a copy of the insurance card must be attached. These measures make sure we get care for a sick or injured youth as quickly as possible.

2009-10 Youth Ministry Activity Permission Form
This is an important document that we use for many of our events (especially when we travel over 2 hrs from home or for any overnight trip). This form can be filled out for any event. It lists the guidlines for each trip and the discipline policy we implement. Both the youth and a parent are asked to sign that they have read and agree to the guidelines and discipline policy.
